Time for A Heart Health Check-up! 
Did you know having uncontrolled high blood pressure can lead to heart disease, causing an enlargement of your heart; which could lead to heart failure? 
High blood pressure is called the slient killer, because individuals are not being screened or treated for their high blood pressure. 
Other risk factors for Heart Disease include: 
· Overweight/Obese 
· Chronic Disease (eg. Diabetes) 
· Elevated cholesterol 
· Family History of heart disease 
· Smoker

· Blood Tests (eg. Cholesterol screening)

· Collection of Family History (eg. Sudden cardiac arrest) 
· Collection of Social History (eg. Smoker)
